ALP: what your level actually means

The short answer: Alkaline phosphatase (ALP) normally runs between about 44 and 147 U/L in adults, though it's important to know that this enzyme also comes from bone, so growing children and teenagers naturally run higher. A high ALP can point to a liver or bile duct problem, or simply reflect active bone growth or healing, while a low ALP is less common and usually not a major concern on its own.

What ALP tells you
ALP is an enzyme made mainly by the liver and bones, which is why doctors often order a related test (GGT) alongside it to figure out which organ is behind an abnormal result. Common reasons it rises
- Bile duct blockage — gallstones or strictures raise ALP as bile backs up.
- Liver conditions — certain liver diseases push ALP upward.
- Bone conditions — including Paget's disease or healing fractures, raise ALP through bone turnover.
- Normal growth in children and teens — active bone growth naturally elevates ALP.
- Pregnancy — the placenta produces its own ALP, raising levels in later pregnancy.
And reasons it runs low
- Malnutrition — including severe deficiencies of certain nutrients.
- Some inherited conditions — such as hypophosphatasia, a rare genetic disorder affecting bone enzyme activity.
- Zinc or magnesium deficiency — these minerals are needed as cofactors for the enzyme to work.
