The short answer: LDL cholesterol measures how much cholesterol is packed inside LDL particles. ApoB counts the particles themselves, because every LDL, VLDL, and Lp(a) particle carries exactly one ApoB molecule. Below about 90 mg/dL is generally considered desirable for ApoB.

The two usually agree, and a routine lipid panel is still the common starting point. They diverge when particles are small and cholesterol-poor: LDL cholesterol can look reassuring while the particle count stays high.
