Estradiol: what your level actually means

The short answer: Estradiol in premenopausal women swings widely across the menstrual cycle, running roughly 15 to 150 pg/mL in the earlier follicular phase, surging as high as 350 pg/mL or more just before ovulation, and settling into the 50 to 150 pg/mL range afterward. Men and postmenopausal women normally run much lower, often under 40 pg/mL and under 10 pg/mL respectively. Because timing matters so much, this result is only meaningful when read against the cycle day, menopausal status, and sex it was drawn for.

What Estradiol tells you
Estradiol is the main form of estrogen, the primary female sex hormone, made mostly by the ovaries in women and in smaller amounts by fat tissue and the testes in men. It drives the buildup of the uterine lining and triggers ovulation at its mid-cycle peak, which is exactly why its normal range changes dramatically depending on where someone is in their cycle. Common reasons it rises
- The normal mid-cycle surge before ovulation — the expected, physiological reason for a high reading in a reproductive-age woman.
- Ovarian cysts or certain ovarian tumors — can produce excess estradiol outside the expected cycle pattern.
- Fertility medicines — ovulation-stimulating treatments intentionally raise estradiol as part of how they work.
- Liver disease or obesity — can raise estrogen levels indirectly by affecting how the body processes and stores hormones.
And reasons it runs low
- Menopause or perimenopause — declining ovarian function is the most common reason estradiol drops in women over time.
- Primary ovarian insufficiency — the ovaries stop producing normal amounts of estrogen earlier than expected.
- Excessive exercise or very low body weight — can suppress the hormonal signals that drive estradiol production.
- Certain pituitary or hypothalamic conditions — can reduce the signals that tell the ovaries to produce estrogen.

Why does estradiol have such a wide normal range?
Because estradiol rises and falls dramatically across the menstrual cycle, a single 'normal range' has to cover the low early-cycle phase, the high pre-ovulation surge, and the moderate post-ovulation phase all at once. That's why the cycle day a sample was drawn on matters as much as the number itself.
Is a low estradiol always a sign of menopause?
Not always. While declining estradiol is the hallmark of perimenopause and menopause, low levels can also come from excessive exercise, very low body weight, or certain pituitary conditions in younger women. Age, symptoms, and other hormone levels like FSH help clarify which explanation fits.
