Weight Loss Is the Intervention With the Strongest Evidence
Among everything studied for fatty liver, gradual weight loss has the most consistent evidence for actually reducing liver fat and improving related enzyme levels. Research generally points to meaningful improvement in liver fat with a loss of around five percent of body weight, with more pronounced improvement in inflammation and early scarring at closer to seven to ten percent, achieved gradually rather than through rapid or extreme restriction.
The amount of weight loss that helps varies by person and starting point, and rapid, severe calorie restriction can sometimes backfire by increasing liver stress in the short term. A gradual, sustained approach tends to outperform crash approaches for this particular condition.

The Sugar and Fructose Story
Added sugar, and fructose in particular, is metabolized largely in the liver and is more directly linked to fat accumulation there than starch or naturally occurring sugars eaten with fiber, such as those in whole fruit. Sugar-sweetened beverages are the single food category with the clearest association with fatty liver risk, because liquid sugar is absorbed quickly and in larger amounts than most people would eat as solid food.
This does not mean fruit is the problem. Whole fruit comes with fiber, water content, and a slower absorption curve that behaves very differently in the body than the same sugar delivered in a sweetened drink or dessert. Reducing sweetened beverages tends to be one of the higher-yield, most sustainable changes people can make.

Coffee's Genuinely Favorable Evidence
Coffee is one of the few dietary items with real, repeated evidence of a favorable association with liver health, including in people with fatty liver. Multiple observational studies link regular coffee consumption, typically two or more cups a day without excess added sugar or cream, with lower rates of liver scarring progression, though researchers have not pinned down the exact protective compound.
This is an association from observational research rather than proof that adding coffee will reverse existing damage, and it does not mean more is always better, especially for people sensitive to caffeine's effects on sleep, anxiety, or blood pressure. Still, it is a rare case where a commonly enjoyed food or drink comes with genuinely favorable data rather than hype.

Building the Rest of the Pattern
Beyond weight loss and sugar reduction, patterns rich in vegetables, whole grains, legumes, and unsaturated fats such as olive oil, and lower in processed meat and refined starch, are associated with better liver fat outcomes in research. Regular physical activity, including both aerobic exercise and resistance training, appears to improve liver fat somewhat independently of weight change.
SGPT, also called ALT, is one of the more common ways clinicians track progress alongside imaging, since it often falls as liver fat and inflammation improve. A single reading fluctuates with recent alcohol, exercise, and illness, so trends over months matter more than any one number.
