Two Conditions, Very Different Urgency
Heat exhaustion develops gradually after hours of heat exposure, heavy sweating, or hard exertion in warm conditions, producing heavy sweating, weakness, headache, nausea, and a fast, weak pulse, but the person is still alert and sweating normally. It's uncomfortable and needs attention, but it is not yet a life-threatening emergency.
Heat stroke is what happens when the body's cooling system fails entirely, and body temperature can climb above 106°F within minutes. It is a true emergency requiring immediate care, not a more severe version of the same discomfort that can simply be waited out with rest and water.

The Sign That Changes Everything
The sign that changes everything is the mind: confusion, slurred words, stumbling, agitation, or collapse in someone who has been in the heat means heat stroke until proven otherwise — call emergency services and start cooling immediately. Hot, dry skin is a common feature but NOT a requirement: people with exertional heat stroke, especially athletes and outdoor workers, are often still sweating heavily. Never wait for dry skin to act.
A person who is confused, stumbling, or not making sense in the heat needs emergency care called immediately, even if they insist they're fine. Heat stroke can progress from early warning signs to organ damage within a short window, so hesitation to call for help costs valuable time.

Cooling Priorities
While waiting for help to arrive, move the person to shade or air conditioning, remove excess clothing, and cool them as aggressively as possible: cold water immersion where feasible, or wet cloths and fanning combined with ice packs at the neck, armpits, and groin where blood vessels sit close to the skin.
Speed of cooling matters more than method, so use whatever is available immediately rather than waiting for an ideal setup. Offer sips of water only if the person is fully alert and able to swallow safely; never give fluids to someone who is confused or unconscious.

Who Is Most Vulnerable
Older adults, infants and young children, people with chronic health conditions, and anyone taking medications that affect sweating or heart rate face higher risk from heat, often with less obvious warning signs before things turn serious. Outdoor workers and athletes training in high heat and humidity are also at elevated risk.
People without reliable air conditioning during heat waves, and anyone left in a parked vehicle even briefly, face particularly steep risk since temperatures in an enclosed car can climb far faster than outside air. Checking on vulnerable neighbors and relatives during extreme heat is a meaningful protective step.

Myths About Beating the Heat
Drinking alcohol or very cold beverages to "cool off fast" doesn't meaningfully lower core body temperature and, with alcohol, actually worsens dehydration. Similarly, pushing through a workout or chores in extreme heat "to build tolerance" doesn't build any real resilience — it just raises the odds of heat illness that day.
Salt tablets without medical guidance and ice baths for otherwise mild heat exhaustion are both unnecessary steps that can cause their own problems; steady fluids, rest in a cool space, and paying attention to changing symptoms cover most situations well. Save aggressive cooling measures for the genuine emergency of heat stroke.
