D-dimer: what your level actually means

The short answer: D-dimer is usually reported as normal below about 0.5 µg/mL FEU, and its main value is what a low result rules out rather than what a high one confirms. A normal D-dimer, combined with a low clinical suspicion, makes a blood clot such as deep vein thrombosis or pulmonary embolism unlikely enough that further clot-specific imaging often isn't needed. A high D-dimer, on the other hand, does not mean a clot is present — it simply means clotting activity happened somewhere, which has many possible explanations.

What D-dimer tells you
D-dimer is a small protein fragment left behind when the body breaks down a blood clot. Because the body is constantly forming and dissolving tiny clots as part of normal repair, D-dimer is designed to be a highly sensitive but not very specific test — excellent for ruling clots out when low, but not proof of a clot when high. - Recent surgery or injury — normal tissue healing after any procedure or trauma raises D-dimer for days to weeks.
- Pregnancy — D-dimer rises progressively through pregnancy as a normal physiological change.
- Infection or inflammation — conditions like pneumonia or sepsis can activate clotting pathways and raise the level.
- Older age — D-dimer tends to run higher in older adults even without any clotting problem, which is why age-adjusted cutoffs are sometimes used.
And reasons it runs low
- No significant clot activity — a normal D-dimer is reassuring and is the main reason this test is ordered in the first place.

Does a high D-dimer mean I have a blood clot?

How useful is a normal D-dimer result?
