What Osteoarthritis Is
Osteoarthritis develops as the smooth cartilage that cushions the ends of bones within a joint gradually wears down over years, reducing the natural shock absorption between bones and eventually leading to pain, stiffness, and sometimes bony changes around the joint. It's often described as a wear-and-tear process, though biological factors beyond simple mechanical use also play a role.
This is a mechanically and biologically different process from inflammatory forms of arthritis, such as rheumatoid arthritis, which are driven primarily by immune system activity rather than gradual cartilage breakdown.

Who Gets It and the Joints Most Often Affected
Age is the strongest risk factor for osteoarthritis, and it becomes substantially more common after fifty. Excess body weight, previous joint injury, repetitive joint stress from certain occupations or activities, and genetics also raise the risk of developing it in specific joints.
The knees, hips, hands, and spine are the joints most commonly affected. Knee and hip osteoarthritis tend to cause pain with weight-bearing activity, while hand osteoarthritis often causes stiffness and visible bony enlargement at the finger joints.

Symptoms and How It's Diagnosed
Typical symptoms include joint pain that worsens with activity and improves with rest, stiffness that is usually brief in the morning, a grating or crackling sensation with movement, and, over time, some loss of flexibility in the affected joint. Unlike the prolonged morning stiffness of inflammatory arthritis, osteoarthritis stiffness usually eases within about thirty minutes.
Diagnosis is usually based on the pattern of symptoms and a physical exam, supported by an X-ray that can show narrowed joint space and bony changes. Blood tests aren't used to diagnose osteoarthritis itself but may be checked to rule out inflammatory causes when the picture is unclear.

The Management Landscape
There is no treatment that regrows lost cartilage, so management focuses on reducing pain and preserving function. Weight management for weight-bearing joints, regular low-impact exercise, and physical therapy to strengthen supporting muscles are foundational and genuinely effective for many people.
Pain relievers and anti-inflammatory medications, topical treatments, and occasionally injections into the joint can help manage flares of pain. For joints with advanced damage that significantly limit daily function despite these measures, joint replacement surgery is a well-established option that can meaningfully restore mobility.

Living With Osteoarthritis, and the Outlook
Osteoarthritis tends to progress slowly over years rather than suddenly, and its pace varies considerably between individuals and even between joints in the same person. Staying physically active, despite the instinct to rest a painful joint, generally helps rather than harms, since supporting muscle strength protects the joint.
An orthopedic specialist or rheumatologist can help when symptoms are significant or the diagnosis is uncertain, and most people manage well for years with a combination of activity, weight management, and medication before more invasive options are ever needed, if they're needed at all.
