What Chronic Kidney Disease Is

Chronic kidney disease means the kidneys have sustained gradual, lasting damage that reduces their ability to filter waste and excess fluid from the blood. It develops slowly over months to years, and because the kidneys can compensate for a long time, many people have no symptoms until a substantial amount of function has already been lost.

The most common underlying causes are long-standing diabetes and high blood pressure, both of which damage the small blood vessels inside the kidneys over time, though other conditions such as chronic kidney inflammation or inherited kidney disorders can also be responsible.

How the Stages Are Defined

Doctors classify chronic kidney disease into five stages based mainly on estimated glomerular filtration rate, or eGFR, a number calculated from a creatinine blood test that estimates how well the kidneys are filtering. Higher eGFR numbers reflect better kidney function, and the stages run from stage 1, with normal or near-normal function alongside other evidence of kidney damage, to stage 5, where the kidneys have very little filtering capacity left.

Alongside eGFR, doctors also check for protein in the urine, since its presence is an important marker of kidney damage even when eGFR is still relatively preserved, and it factors into staging and risk assessment as well.

Symptoms, and Why Many People Don't Notice Early Stages

Early chronic kidney disease is often symptom-free, which is why routine blood and urine testing, particularly in people with diabetes or high blood pressure, is the main way it gets caught early. As function declines further, symptoms such as fatigue, swelling in the legs or around the eyes, changes in urination, poor appetite, and difficulty concentrating can appear.

Because the kidneys play a role in regulating blood pressure, red blood cell production, and bone health, more advanced chronic kidney disease can also contribute to anemia and bone-related complications, which is why creatinine and eGFR monitoring extends beyond just tracking the kidneys themselves.

Slowing Progression: What Actually Helps

The most effective strategies for slowing chronic kidney disease progression are tightly controlling blood pressure and, when present, blood sugar, since both are the leading drivers of ongoing kidney damage. Specific classes of blood pressure medication have additional kidney-protective effects beyond just lowering blood pressure and are often preferred for this reason.

Limiting certain medications that can stress the kidneys, particularly some over-the-counter pain relievers when used regularly, avoiding smoking, and following dietary guidance on sodium, protein, and potassium tailored to the individual's stage all play a meaningful supporting role.

Living With Chronic Kidney Disease, and the Outlook

Regular monitoring of creatinine, eGFR, and urine protein allows a doctor to track the trajectory over time and adjust treatment before more damage accumulates. A nephrologist typically becomes involved as the disease advances, or earlier if the cause is unclear or progression is faster than expected.

For most people diagnosed at an earlier stage, consistent blood pressure and blood sugar control can substantially slow further decline, and many live for years with stable, manageable kidney function. In advanced stages, dialysis or kidney transplantation become part of the conversation, but that outcome is far from inevitable when chronic kidney disease is caught and managed early.
