Why Total Cholesterol Alone Falls Short
Total cholesterol adds up LDL, HDL, and a portion of triglycerides into a single number, but this can be misleading because LDL and HDL move risk in opposite directions. A high total number driven mostly by high HDL is a very different picture than the same number driven by high LDL.
This is why clinicians look at the individual components rather than relying on the total alone. Two people with identical total cholesterol numbers can have meaningfully different cardiovascular risk once their LDL, HDL, and triglycerides are broken out separately.

LDL and HDL, in Plain Terms
LDL carries cholesterol toward tissues and, when levels are high, contributes to plaque buildup in artery walls over time, which is why it is often referred to informally as the less favorable type. Lower LDL is generally considered better for cardiovascular risk, within reason.
HDL works in something closer to the opposite direction, helping carry cholesterol away from artery walls back to the liver for processing. Higher HDL, up to a point, is generally associated with lower cardiovascular risk, which is why it is often called the more favorable type.

Where Triglycerides Fit In
Triglycerides are a different kind of fat in the blood, largely reflecting recent food intake, especially sugar, refined carbohydrates, and alcohol, alongside body weight and activity level. High triglycerides alongside low HDL is a pattern that shows up often in metabolic syndrome and insulin resistance.
Because triglycerides respond fairly quickly to recent meals, this test is usually done fasting to get a more stable, comparable reading. A single non-fasting result can be noticeably higher than a fasting one from the same person on a different day.

Why Ratios Add Useful Context
Some clinicians also look at ratios, such as total cholesterol to HDL or LDL to HDL, as an additional way to summarize the balance between favorable and less favorable components in one figure. A ratio can flag risk that a single number might understate on its own.
